No Rate Change for 2009-10

Currently Enrolled Employees: This is the time for you to change plans or change the dependents on your plan.  If you do not wish to make changes no action is necessary and you will be renewed in your current plan at the new rate.

New Enrollments:  New full-time faculty must enroll in a SISC medical plan.  Part-time faculty may enroll if eligible.  To be eligible for benefits part-time faculty must have a 50% or greater assignment.  Generally, it takes 7.5 units of lecture to reach 50%.  If you are interested in enrolling but unsure of your assignment percentage please contact the Benefits Office at 546-3161 or email kdecarli@cuesta.edu to check your eligibility.

Enroll for the First Time Fill out New Enrollment Form
Submit to the Benefits Office by Sept 1
Change Plans Mark your selection on the Plan Change Form
Submit to the Benefits Office by Sept 1
Add Spouse or Domestic Partner Complete a Change Form
Provide a copy of marriage license or Declaration of Domestic Partnership
Submit to the Benefits Office by Sept 1
Add Dependent Child Complete a Change Form
Provide a copy of the birth certificate
For dependents age 19-25 provide proof of student status or IRS dependency
Submit to the Benefits Office by Sept 1
Cancel Dependents Complete a Change Form
Submit to the Benefits Office by Sept 1
Keep the Same Plan & Dependents No action is necessary.  You will be re-enrolled in the same plan.
