Request for Vision to be used during Open Enrollment Period
or for a qualifying event (marriage, birth of a child, delete a dependent).
This form should be returned the the benefits office.
Medical Eye Services does not
provide ID cards. You simply need your Group Number and Social Security
Number to access your benefits. For your convenience, you may use the
Vision
ID Card form to create your own ID
card.
If possible, take a
Claim Form with you to the
doctor's office. This will make it easier for the staff to process your
benefits.
