Effective January 1, 2007, every
employee who is eligible to receive a District contribution for the health and
welfare benefits program (fringe) is required to enroll in a minimum of single
coverage with the District's medical plan.
The following are links to the three Blue Shield plans for Classified, Management and Confidential Employees:

Subscriber's Statement of Claim form is used only when the provider of
service does not submit your claim directly to Blue Shield.
Subscriber Change Request is used to change address, add or delete dependent, etc. This form must be
turned in to the Insurance/Benefits Office. Do not send directly to Blue
Shield.
Student
Certification is needed for dependents over age 19. Blue Shield
will request student certification annually.
