Effective January 1, 2007, every
employee who is eligible to receive a District contribution for the health and
welfare benefits program (fringe) is required to enroll in a minimum of single
coverage with the District's medical plan.

OPEN ENROLLMENT
Subscriber Change Request is used to change
plans, address, add or delete dependent, etc. This form must be turned in to the
Insurance/Benefits Office.
Subscriber's Statement of Claim form is used only when the provider of
service does not submit your claim directly to Blue Shield.
Student
Certification is needed for dependents over age 19. Blue Shield
will request student certification annually.
