To be eligible to
pre-designate an employee must be a permanent employee with 50% or
more assignment.
Print the three documents
below and take to your personal physician.
This information must be
in your personnel file 30 days prior to an injury in order to be in
affect.

Schools Insurance Program for Employees (SIPE). Receive a
