For specific information on hours or to make an appointment call the San Luis Obispo Campus center at (805) 546-3171, or the North County Campus center at (805) 591-6200, x4207. * Not all services are available at the North County at this time but all students can use services on either campus. |

Cost to Student:The Student Health Services Program is
supported by the college’s Health Fee, paid at registration. All registered
students are eligible to receive services. |

Confidentiality:All medical and counseling visits and records are confidential with legal constraints, and are not part of the student’s academic records. Records will not be released without the student’s written consent. |

|
Student Under 18 years old Any student under the age of 18 years will be required to have a parental consent form signed before receiving treatment except in emergency situations or when the case is exempt by state law. |

Accident Reporting and Insurance:Cuesta College has limited supplemental Student Accident Insurance for injuries sustained during school-related accidents and activities. If you have other medical insurance coverage, all benefits under that policy must be fully utilized before the Student Accident Insurance policy applies. The Student Accident Insurance company will determine the eligibility of all submitted claims. Information bulletins are available upon request in the Student Health Center. It is the student’s responsibility to report accidents to the Student Health Center within 72 hours. Health Education:General health and mental health awareness events are sponsored throughout the year. Call for specific information at (805) 546-3171. A wide variety of health education and wellness literature is available at the Student Health Center. |
