High School Transition

Thank you for your interest in DSPS. All students must complete a Cuesta College Application, an Application for DSPS Service, a Consent for Release of Information and have appropriate disability verification on file with the DSPS office to qualify for services.

You may access the forms needed below. Send completed and signed forms, along with the required documentation reports to:

Upon receipt of the application packet, a disability specialist will review the packet and you will be notified of your status by mail within two weeks. (NOTE: The response time will be longer during semester breaks and summer.)
