Verification of Disability

Cuesta College DSPS Department will send copies of your disability-related information to others if you request it and have a current SEC on file.

Verification of Disability for EOPS

Procedure

1. Student completes and signs Request for Information to be Sent form or the EOPS Verification of Disability Form. Note: EOPS, Cuesta College must appear in the "Send to" Name field. 
2. Student submits request to DSPS.
3. Completed DSPS verification form may be given to student or mailed to EOPS. The preferred method is to have the student wait while the form is completed.
4. The student delivers Verification form to EOPS.
